Need help with my Sorc

Hi,
I'm playing Stormweaver Sorc and following Archmage Spark Lightning Conduit Stormweaver Build Guide from Maxroll (created by Crouching_Tuna). I'm currently 94lvl with some decent gear (I guess). I tried to do some changes to my passives by myself but something doesn't feel right. I'm doing T15 maps pretty comfortably but T3 Breach Boss one shotted me and it felt like I wasn't doing that much dmg to him.
Can you help me with some suggestions? I have prepared a planner with my current setup with the exact gear I'm using and the passives:
